What are Direct Primary Care (DPC) Memberships?
Direct Primary Care is a healthcare model where patients pay a flat monthly membership fee for unlimited access to comprehensive primary care services. There’s no insurance billing, no copays, and no surprise bills – just high-quality, accessible care when you need it.
$175/month per person – all ages
$150/month per additional immediate family member
If you don’t have insurance, or your insurance plan is out of network, DPC can work great for you!

Forms

Medication refills & prescriptions
- Prescriptions will be sent to your pharmacy of choice or provided to you at the time of visit
- All medication request will be processed within 24 hours
- Urgent refills will be sent out immediately
Lab testing
- Skin, nail, urine, and stool samples will be collected at the time of visit
- Blood tests will be completed at Quest Diagnostics
- We offer discounted rates for DPC patients
- Blood testing panels are different and we provide you with upfront pricing before the orders are placed – ranging from $7-$200.
Terms of Membership
- Your membership is on a month by month basis. No commitment required.
- You may cancel your membership any time, but a written notification 30 days prior to the upcoming scheduled payment due is required. The notice can be sent via messaging through the Patient Portal or email.
- If you have cancelled your membership, but would like to return to us, you will be required to pay a one-time $250 re-enrollment fee (non-refundable).

Small Business Memberships
We provide full service primary care as described above to employees of small businesses. We provide small companies a chance to offer inexpensive and comprehensive health benefits to employees. This membership will apply to your business only if we are not contracted with your health plan. We use multi-layer pricing with multiple options to help you choose what best fits your employees needs. Please email us to set up a phone call.
